Insurance and Rates | Sycamore Chiropractic & Nutrition

We currently accept the following insurance plans:

  • Anthem
  • Medical Mutual of Ohio
  • UMR
  • UnitedHealthcare (UHC)


Medicare

While we are not participating providers with Medicare, we welcome Medicare patients. You will be charged Medicare's allowable rate, and we will provide all necessary documentation for you to submit a reimbursement request to Medicare.


Medicaid

We do not participate with any Medicaid plans. Patients with Medicaid coverage will be treated as self-pay (cash) patients.


Anthem TriHealth Plans

We do not participate with any Anthem TriHealth plans.


Self-Pay Options

Affordable self-pay rates are available. Please contact our office for current pricing or with any questions regarding your insurance coverage.
